Here you will find information about how to refer to all of our services along with copies of referral forms.
Alice House Hospice provides specialist palliative care to all adults over the age of 18 who live across the Hartlepool area and within East Durham.
Professionals wishing to refer to our range of services can complete one of our attached referral forms indicating the service required. The completed referral form provides us with the information required to process and prioritise the referral and allows us to identify patient care needs.
We aim to acknowledge referrals received within 2 hours of receipt Monday-Friday, 9am-5pm.
